Well, it wasn't 100% medically accurate. I feel like if it had been, the whole fic would have been a lot more horrifying because it would have rung with the truth of reality. You'd have to go to great lengths to keep a pony alive while mutilating it, and Pinky Pie just didn't do the things she would have had to do.
I think the joke, though, is that it's almost darker to read the fic and have that be your only thought. "It was alright, but it could have been more sinister if written with better surgical knowledge," is probably not the healthiest first instinct.

@filleg: I was the same way. Read the story, just sat there for a good half-hour after finishing it, thinking "wtf did i just read?". It's not scary, just really disturbing, partially because pinkie's in character the whole time.
